OSHA Recordable

OSHA recordable incidents are work-related injuries, illnesses, and fatalities that must be recorded and reported by employers. Injuries and illnesses to be disclosed are work-related occurrences’. This includes exacerbating a pre-existing injury or illness.

Severe injuries and illnesses must be reported directly to the OSHA in addition to being recorded by the employer
All work-related deaths
All in-patient hospitalizations, amputations and loss of an eye.
include instances that require surgery, prescription medication, casts, or stitches.
certain fractures, measurable hearing loss, and chronic disability

Occurrence’s where the employer must record an incident on the appropriate documents.

job transfers, or medical care beyond first aid
Illnesses associated with workplace exposures, such as some cancers, must also be recorded.

Must Report Directly to OSHA

The OSHA’s recording standards mandate that severe injuries and illnesses must be reported directly to the OSHA in addition to being recorded by the employer.

All in-patient hospitalizations, amputations and loss of an eye must be reported within a 24-hour period.

Events that should not be reported to OSHA or put in to the OSHA log.

Opt out

Small company’s have the option to not participate in the reporting process except for severe injuries or illnesses

Exemptions
10 or fewer employees (entire company)
10 or less at all times during entire year
Still must report fatalities & catastrophes

If the employee independently and voluntarily requests that his or her name not be entered on the log
an injury or illness to an intimate body part or to the reproductive system.
an injury or illness resulting from a sexual assault.

a mental illness.
a needlestick injury or cut from a sharp object that is contaminated with blood or other potentially infectious material

Privacy Reporting Techniques

OSHA 300 log enter privacy instead of a name
Keep separate confidential list of case numbers and employee names
You may use discretion when describing the injury
OSHA 300 and 301 you may use discretion when describing injury.
a. enough information to identify the cause of the incident and the general severity of the injury or illness.
b. Serious outcome must be recorded for each case
