Assignment content

For this project you will be assigned a team and an accident scenario posted to black board.

1. Your task is to fill-in a set of OSHA documents with the information you learned about an employee injury.
2. Each team will be assigned an accident scenario posted on Brightspace, discussion tab, OSHA reporting project forum. Use the assigned thread for any content you add.
3. Download the OSHA forms fillable with instructions document. Open it and read it. Fill in the forms to the best of your ability. Each student will need to turn in the filled in documents on Brightspace in the assignments tab, , OSHA reporting project assignment.(Put the names of all the students who worked on the project together in the header)
4. Create a short video or ppt to present to the class at our next meeting of how your team interpreted and reported the incident. The PowerPoint file or video link should be posted on Brightspace in the discussion tab. Be sure to post the names of every student on your team in the same thread.
5. Do not turn the forms into OSHA.

Instructions for filling in the form

1. Download and open inside adobe acrobat to turn on all form features
2. Save form as you work.
3. Fill in the optional Worksheet to help fill out the summary first to have the form correctly calculate the number of full time hours worked.
4. Fill in the OSHA’s Form 300 (Rev. 04/2004)Log of Work-Related Injuries and Illnesses sheet.
5. The form OSHA’s Form 300A (Rev. 04/2004) Summary of Work-Related Injuries and Illnesses  should auto populate. If not fill in the information you learned from the OSHA form 300.
6. You will need to fill in the establishment information section on the OSHA form 300A. The NAICS number for Entertainment is 711111.
7. Fill in the Optional Calculating Injury and Illness Incidence Rates using the number of injuries listed in your scenario and the total hours calculated in the optional Worksheet to help fill out the summary sheet.
8. Fill in the OSHA’s Form 301 (Rev. 04/2004) Injury and Illness Incident Report using what you learned by filling in the form and the scenario you selected.

Assignment turn in information

1. Turn in the filled in version of the OSHA reporting document pdf to Brightspace, assignments tab, OSHA reporting project icon.
2. Turn in a ppt to present your document and compare your DART rate to the industry Dart Rate. You can look up the industry info online. Discuss what you learned to Brightspace, discussion tab, OSHA reporting project forum, use your assigned scenario thread.
3. A helpful website to compare your DART rate to the industry Dart rate
