Reflection is an important part of place-based learning. It helps transform a field trip into an opportunity of deep learning. City Tech Field Trip Forms

The documents needed to complete before participating in a field base learning opportunity with students is listed here.

If you are meeting with students within the five boroughs use this form: Field Trip Request [Five Boroughs]

If you are meeting with students within the five boroughs and the student is under 18 years of age, use this form in addition to the basic field trip form listed above:  Class Trip Release [Five Boroughs – Minor Approval]

If you are providing transportation with in the five boroughs or if your are traveling outside of the five boroughs, use this form in addition to the basic field trip form listed above: Class Trip Release [Five Boroughs – Minor Approval]
