My community health nursing rotation took place at Kings County Hospital Center (KCHC) in Brooklyn, NY. The objective of this rotation was to collaborate with other group members and KCHC staff in assessing the prevalent health problems of the communities served by KCHC.Upon completion of the assessment, group members were to engage in a Service Learning Project in which we prepared a health-promotion teaching plan. We found that the communities served by KCHC have high hypertensive and diabetic populations. I participated in a health seminar in which we discussed hypertension and diabetes, conducted health teaching, and encouraged health behavior change.
