- Perform assessment visits of clients in their home; initiate and conduct necessary revisions of clients plan of care.
- Collaborate with Case Managers and Social Workers of the Managed Long Term Care/MCO companies to ensure that clients are adequately serviced and resources are provided according to their needs.
