What does the term vulnerable population mean?

The term vulnerable population describes individuals and groups of people who have health issues where because of certain factors like age, gender race or sexual orientation they do not reap the full benefits of the health care system (http://www.ajmc.com/journals/supplement/2006/2006-11-vol12-n13suppl/nov06-2390ps348-s352).
What is the relationship of social and economic factors on health and wellbeing in infants and children? People living with HIV and Aids. The elderly and pregnant woman?
Children are at risk for many safety and health related problems. Children from poor communities are at risk for many problems including malnourishment, accidents and diseases from lack of immunization. People with Hiv and Aids have to deal with the social stigma placed on their disease, the disease tends to affect gay minority men the most. The elderly are susceptible to many health problems affected by social and economic factors including funds for medicine and safe living conditions they are also exposed to potential elder abuse (D Amico and Barbarito, 2016, P 104). Pregnant woman who are from low income populations may have issues with pre natal care and education on proper health care during pregnancy and after.
What are two area codes or neighborhoods in Brooklyn that reflect the greatest health disparities?
According to http://www.nydailynews.com/new-york/brooklyn/brooklyn-health-statistics-reveal-inequality-borough-article-1.2397280 the two areas of Brooklyn that have the highest health disparities is the area of Brownsville and Bedstuy, the correlation was based off of finances and life expectancy which showed health problems higher in those neighborhood including diabetes and obesity .

Part 2.
Case scenario: During an interview as a triage nurse in an urgent care clinic or emergency room, (choose one) a child, a teen, a young adult, presenting with vague abdominal pain and fearful demeanor. They are accompanied by a person who is with them claiming
to be a relative

What are the areas that you would consistently want to conduct a physical assessment and obtain a health history?
A young adult is complaining of abdominal pain and is fearful in demeanor, the first thing I would do is inspect the abdomen for any signs of injury or trauma and palpate the location where the pain is at. I would ask the individual if there was any vomiting or diarrhea and what was eaten in the last 24 hours. I would also ask when the pain started and what they think may be the cause of the pain. I would also ask about any potential drug use, suicidal behavior or sexual activity including possible pregnancy. I would consistently assess for signs of trauma considering MVA are the top illness and death for this age group.

Describe your attitude, philosophy and approach to communicating with patients?

My attitude and philosophy about communicating with my patients is that I want my patients to feel confident in me as their nurse. I approach my patient by stating their name and introducing myself to them as their nurse, Mr. Douglas. I ask questions during my assessment and I make sure I explain to them what I am about to do before I touch them or perform any therapies.

Describe what you think is the most important aspect of communicating with patient?

I think the most important aspect of communicating with a patient is clarity in the communication process. When I speak clearly and the patient also speaks clearly there is little room for miscommunication or misunderstanding.

What do you find are YOUR most challenging aspects of communicating with patients, families or colleagues?

My most challenging aspect of communicating with patients is when they speak a different language and an interpreter system has to be used. For families it is when multiple family members are in a room asking multiple questions. When it comes to my colleagues the most challenging aspect of communication is when a Colleague shuts down and does not want to hear what I have to say.

What would you most like to improve about YOUR communication style and practice?

I think I can improve in my communication with being more detailed when communicating with my colleagues, which would include using more medical terminology and adding more background information.
