HUS 3605 Child Welfare and Family Services TERM PAPER – ASSESSMENT AND CASE PLANNING SUMMARY Due Date: Session 12 Choose a young adult (ages 19-21) and discuss the following aspects countered as a Child and growing up. Physical, Cognitive, Psychological) development/functioning impairments/strengths Academic, behavioral) performance/functioning Family, Social/Peer) relationships/conflits stressors/support Be sure to get the consent of the young adult before the interview.  Explain the purpose of this class project and the confidentiality of the assignment. Plan to have a series of interviews (3+) with the young adult. Include a statement on page one that all identifying information about the interviewee and his/her family have been disguised or changed in this written assignment. Guidelines To gather information and complete a culturally competent assessment and case planning summary. Organize and summarize all collected data, using the headings suggested by Instructor. The discussion of relevant cultural issues should be included throughout the assessment and case plan. The term paper should include a title page, be double-spaced and 8 to 10 pages not including cover page and reference sections in length. Proofread and edit the term paper before submitting it to the instructor. If needed, go to the Writing Center (AG 18) for help in revising the paper. Grading: Content (50%) Organization, Grammar & Punctuation (50%) Late work without attached medical (or approved) documents will have 10 points deducted for each week past the due date.
