Whitening

This patient, a young 18 yr old female wanted whitening even though she already had a nice bright shade. The beginning shade was B1 and the after shade was A1. The whitening system we used was Colgate Optic White, it takes ten days on average and provides a gentle yet effective way for patients to whiten at home.

This section encapsulates some of the interesting cases I’ve seen in clinic, and an important lesson I’ve learned from each.

Case Study #1– The Rheumatoid and Psoriatic Arthritis Patient

This patient had rheumatoid and psoriatic arthritis as well as anemia. This was an interesting case because the iron supplements this patient takes caused staining on the anteriors (mainly in the lingual area). I’ve seen patients come with staining on their teeth and I did not realize initially that it was the iron that was a big culprit! After looking into this further, research suggests that the iron reacts with bacteria present and causes some staining. Now I see the connection between the medications a patient takes- I am way more mindful of oral manifestations of medications!  This patient also had arthritis which meant the standard simple oral home care of flossing and brushing was not as easy for them. We worked together to choose oral homecare instruction that this patient felt comfortable with and would be sustainable for them.

Case Study #2 – The Cardiac and Skin Graft Patient

This patient  recently had two valve replacement surgeries in Nov. 2020, and came in with no medical clearance. After obtaining medical clearance we had him come in early, take antibiotic prophylaxis and it was time to get rolling. Throughout the appointment he was not very comfortable being put back so I opted to stand rather than sit and try to maintain ergonomics as I scaled. Furthermore, he had a very fascinating skin graft due to a past sarcoma on the anterior portion of his tongue. The most interesting and important treatment plan modification in this case study was avoiding scaling certain areas due to severe mobility and recession. It was definitely a very unique case!
