My experience in Community was very eventful for me. I met with many people in the East-Flatbush community in which I live in.  My clinic site was Kings County Hospital on 451 Clarkson Avenue.  I learned many things about my community in class and in my clinical site. The classwork help me relate to my clinical rotation in a couple of ways. The community research paper was an eye opener to me. I researched my community and learned many things that I found helpful. I learned about the farmer’s market that is held on Clarkson Avenue on the Lawn of Kingsborough medical center every Wednesday from 9am to 2pm. I learned about the wellness center in the neighborhood and I even learned where people go to socialize in my neighborhood. This information that learned in class was used in my clinical rotation. I educated my clients about the neighborhood and some clients even ask for a copy of my community assessment paper, which I brought to my clinical rotation. My clinical and didactic setting together helped me reach my nine objectives for this course.

 

Objective 1: Demonstrates individual professionalism through personal behaviors and appearance

 

 

Professor Finnegan made sure my clinical group demonstrated professionalism through personal behaviors and appearances. We held conference in a secluded area to main the client confidentiality. I assume responsible for my learning by coming prepared to class and to my clinical site, completing my assignments on time, seeking guidance when needed and participating in clinical conferences. I came to my clinical site on time and dress appropiately in Black and white dress attire.

 

 

Objective 2:    Employ analytical reasoning and critical thinking skills when providing care to individuals and families in the community setting.

 

 

My clinical Professor made sure we employed analytical reasoning and critical thinking skills when we provided care to individuals and families in the community setting. We interview clients and nursing records to gather relevant information. We assessed the patient holistically, by assessing cultural, religious, social and physical factors involved.  We prioritized care based on data and applied nursing interventions in a safe timely manner when allowed. We did not administer any medication. Unfortunately we were not able to evaluate the outcomes or modify care because we seen each client only once. We utilize principles of personal safety when working in the clinical setting.

 

Objective 3:    Effectively communicate with diverse groups and disciplines using a variety of strategies regarding the health needs of individuals and families in the community setting.

 

In clinical rotation we effectively communicated with diverse groups and disciplines using a variety of strategies regarding the health needs of individuals and families in the community setting. While interviewing the patient I used therapeutic communication with the clients I spoke to. I use appropriate channels of communication. I also adapted communication technique to the developmental needs of the client.  I communicated clearly and effectively with my instructors and others. I documented assessments and intervention correctly and reported significant data to instructor and healthcare team.

 

Objective 4:  Establish environment conducive to learning and use a plan for learners based on evidence-based practice.

 

I established an environment conductive to learning and use a plan for learners based on evidence-based practice. I met with many people during my clinical rotation.  I established an environment conductive to learning and implemented my teaching plan for my clients in the community setting. I was not able to evaluate the outcomes.

 

Objective 5:  Utilize informational technology when managing individual and families in the community.

 

When researching my community for my paper, I utilized informational technology. I use this information with my clients in the community setting.  I always maintained confidentiality with client records.

 

Objective 6.  Demonstrate .a commitment to professional development

 

I always demonstrate a commitment to professional development.  As a nurse, I always read at least 15 minutes a day. I read current articles, mostly peer review to keep up with current healthcare trends.  I plan to educate myself in nursing and other areas for the rest of my life. I’m committed to adjust to the challenges of independent practices of a community health nurse.

 

Objective 7.  Incorporate professional nursing standards and accountability into practice

When in the health profession, it is important to incorporate professional nursing standards and accountability into practice. As a nurse, I always utilize American Nurses Association standards in clinical practice. I comply with Kings County hospital standard of practice and I’m aware of their mission statement. I know I’m held accountable for my actions as I represent myself, New York Technical College and Kings County Hospital.

 

Objective 8. Collaborate with clients, significant support persons and members of the health care team

 

While in the clinical setting, I collaborated with client and my professor to create plan for my clients.  I collaborated health care teammates to coordinate client-care, identify healthcare resource s, help the client make appropriate lifestyle changes and connect to other community agencies.

 

Objective 9. Recognize the impact of economic, political, social and demographic forces that affect the delivery of health care services

 

One of the main thing s I learned in school and in clinical rotation is that the impact of economic political, social and demographic forces must always be concerned. I acted as an agent to appropriate healthcare resource for my clients. I introduced my clients to the neighborhood wellness clinic. I collaborated with my client to identify solution to complex problems in the community setting.

 

 

I learned many things this semester in class and in my clinical site. I believe this was one of my most challenging semesters due to the Hurricane Sandy and other things. I learned about my community, my clients, and my role as a community healthcare nurse. The Hurricane made me find a new appreciation for disaster readiness. I use the power point given in class on disaster readiness in my clinical rotation
