Prophy Jet – Air-Polishing: Before & After
Age: 45; Male; Medical history WNL; No systemic diseases
CC: Patient complained the brown staining appearance on the border of each anterior tooth, both facial & lingual surfaces.
Patient reported heavy coffee drinker, brushed two times daily, did not use floss or use any mouth rinses. Oral hygiene instruction was given on flossing the interproximal areas (hugging each tooth). Electric toothbrush was recommended for better stain and plaque removal, along with Colgate total dentifrice for gingivitis.
Treatment Plan: Air polished using prophy jet with sodium bicarbonate powders before scaling, the result was astonishing as it showed on the picture. Followed by ultrasonic debridement and hand scales to complete.

Severe Attrition – Natural Tooth against PFM Crowns
Age: 66; Female; Medical history WNL; No systemic diseases
CC: Patient complained missing incisal edges of teeth on the lower anterior sextant five.
Upon assessment, the patient stated having porcelain fused to metal (PFM) crowns and pontic bridge on upper anterior sextant two for over ten years. The patient reported grinding at night time and clenching under stress. Due to the difference in hardness, porcelain 460, whereas enamel 343, research has shown that when grinding and clenching of natural teeth against PFM crowns, natural teeth will present with severe wear over a long period of time.
Treatment Plan: Mandibular nightguard was recommended to the patient, together with proper oral self-care instructions on how to clean around the crowns and under the bridge.
