Case Management/ Referrals

Case management will send out referrals. Sending out referrals will help out the teens and their parents. They also will send them to different places such as hospitals and clinics so that they can get themselves tested. We will also have our case managers send them to clinics such as Planned Parenthood. Also send them to supported health centers in providing preventive care, treatment and screenings. The service offered at Planned Parenthood are Abortion services, birth control, HIV testing, Men’s healthcare, Morning after pill (Emergency contraception), pregnancy testing & services, STD testing, treatment & vaccines, women’s healthcare. In planned parenthood there is free walk-in services to get a free pregnancy tests. The second thing our agency will do is focus on schools. Guidance counselors will be responsible for providing important information to teens such as, locations, telephone numbers to reach doctors. Teens will also be educated on HIV, and STIs. This will help the teens because they would not have to feel lonely. We will hold their hands through everything. Case Managers also provide the location, and contact information of hospitals, and clinics including the right doctor for them to see. This will help teens to know more about safe sex.
