My project that I have been working on is an informational workshop on diabetes. There are many clients who are diagnosed with different types of diabetes and are not aware of the consequences of not following the direction of their physicians’ instructions or just don’t understand the special care instructions they have to follow. Conducting this work shop will give the clients a better understanding of their illness and better care for themselves. Some of the cause of not following the direction of the prescribed medication are high glucose levels, strokes, heart attacks and blindness, etc. This work shop would consist of the medication management, how to properly dispose of needles after each uses, proper types of container to be used for disposal, proper dieting, the cause and effect of not taking medications as prescribed by their doctors’. Informing the client about the good and bad A1C levels. How to keep a good A1C level. How to check glucose level. There will be myself as the Case Manager, the nurse who will be instructing the work shop and my supervisor who is also a registered nurse. My Colleagues will also assist with informing their clients on their case load about the work shop. I have prepared flyers to be handed out to the client who are diagnosed with diabetes. The work shop took place on Friday November 20th,2015 from 12pm-2pm in room 501 located on the fifth floor conference room of our office location. There will be healthy refreshment served during the work shop. Pamphlets will be provided with informative information for the clients on the cause and controlling diabetes. Clients who do not have proper glucose meters will be provided new glucose meters with test strips. We had our deputy director who donated the glucose meters and test strips.
The workshop was such a success that we are going to conduct the next work shop for the clients on Friday November 18th, 2016 for a follow up session. This maybe a continuing on going workshop.
I really did not have any challenges or issues with implementing my project and I enjoyed doing the project because it was a success.
