The field trips forms are attached. Please complete a Field Trip Request form for each trip you plan to take and return the form(s) to Prof. Giuliani/the COMD Chairperson to sign. Updates/cancellations to the request(s) can be submitted later on if necessary.
If you have any students under 18 (eighteen) years of age, or if your trip destination is outside the 5 boroughs, then please have your students complete the Field Trip Release form when class begins.
