Stein Center mission is to cultivate friendly relations among the members of the Stein Senior Center and the general public; to provide services with an emphasis on group growth; to share the strengths, and wisdom of senior citizens; and referral services, and to safeguard the health and wellness of senior citizens.

Sunset Park Senior Citizens is located in the community of 11220. From mere observation, compared to the adjacent luxury homes of Shore Hill, Sunset Park is a middle-class area. According to NYC.Gov/Health, there is an average of 120,400 people residing in Sunset, but this was based of the U.S. census 2000 which means there can be even more people residing in the are. The education levels vary, but which a low percent compared to the rest of Brooklyn, the majority do not have college degrees which is approximated at 15%. It was quite obvious in our clinical and dealing with the clients that according to the NYC.Gov/Health that Sunset Park has the highest proportions of Hispanic and Asian residents if compared to all of Brooklyn. According to the reports of Take Care New York report card though there are many issues the residents in Sunset Park need to address. Goal 1 focuses on whether the resident has a regular doctor or other health care provider which in Sunset Park 31% on average don’t which is higher than the overall average for NYC or Brooklyn which is only 23%. This is a concerning issue which Obamacare will hopefully eliminate, because when people dont have insurance they tend to go to the ER which becomes an expense to society. In regards to health insurance, 4 out of 10 in Sunset Park were without . A second goal in the community is to be Tobacco-free which Sunset Park faces to be on the average with NYC, but it is noted that less (51%) are attempting to quit in comparison to Brooklyn which is 65%. A great distress was the results for the Goal 5 which is Get help for Depression. In Sunset Park 7% on average experience issues yet their percent for treatment for mental illness was amazingly lower than Brooklyn. Reviewing this information gave me insight on the population I am presenting doing my clinical in, but also made me aware of how much people in this community particulary need the health information to improve their own health. It was astonishing for me as a maternity nurse to read that few obtain prenatal care in this community still, especially if they are teenage mothers (15-19) when there are endless, free services that they could utilize. This was Sunset’s best statistic though as the teenagers have been obtaining care and its dropped by 40% for those whom weren’t and only 16% still dont obtain care in comparison to the 27% in Brooklyn overall that don’t. These poor statistics overall make me very aware that during the community clinical how critical it is to educate the clients, because although we can not fix the world we may be able to improve a person’s longevity.

Community Assessment-Bellevue Hospital Center is located at 462 First Avenue, New York, NY 10016. This hospital is a member of the New York City Health and Hospital Corporation, the largest hospital entity in America. BHC was founded on March 31, 1736, and is the oldest continuously operating hospital in the U. S. A. This was four years prior to the birth of George Washington, and forty years prior to the signing of the Declaration of Independence. Bellevue has been serving its community until currently, providing multiple services as incubator for major innovations in public health, medical science, and medical education.
In assessing the service needs of the community it was observed that obesity, diabetes, tobacco use, asthma, hypertension, and high cholesterol are at a higher level in the hospital’s service area than in NYC, as was reported by the NYCDOH. There are eight primary service area neighborhoods-four reveal higher rates in diabetes, five reveal higher rates in high cholesterol, three in hypertension and obesity, and two in tobacco use and asthma.
In this community, a significant problem is noted in healthcare due to the language barrier, seeing that English is not the primary language of a large number of the residents. It is usually difficult to coordinate appropriate, effective healthcare. BHC is known for assessing healthcare needs, planning and coordinating care and providing resources to better facilitate the management of chronic diseases. Outpatient clinics, e.g. diabetes, virology, asthma, & medical are utilized by residents with chronic diseases for follow-up care. BHC also provides a trained multi-disciplinary team who specializes in rendering support to victims and families of the World Trade Center disaster. The interpreter phone system is provided in all clinics to help with the teaching and care needs.
BHC provides healthcare for a large area of NYC communities consisting of Whites and Hispanics, with a significant increase in the Asian population. Though the population is aging, young adults and children make up a larger percent, than in other areas of NYC. Thirty-one percent of families with children, and twenty-two percent of all families in the community are living below federal poverty guidelines.
The neighborhood consists of several Chinese restaurants, McDonald’s, Subway, and pubs. The hospital is accessible by the M15 and #6 train. The neighborhood police precinct is 13th precinct located at 230 E 21st St, and City Fire is the firehouse that serves the community, located at 292 5th Ave.

Approximately 50% of the outpatients (excluding visitors to the emergency room) reside in 27 different neighborhoods. Clients come from Brooklyn and Queens and various neighborhoods in Manhattan. Only 3.21% of the total outpatient visitors (excluding visitors to the emergency room) live within the zip code zone 10016, the zip code area in which Bellevue is located:
“Although BHC is located in the Kips Bay neighborhood on the east side of Manhattan, its primary service area is defined as 27 zip codes that account for 50% of its unique outpatients served from July 1, 2011 through June 30, 2012, excluding individuals who had only an emergency room visit. Bellevue is located at 462 1st Ave, New York, NY 10016. Bellevue is one of the oldest public hospitals in the United States. Bellevue sees close to 800,000 patients a year. The community surrounding Bellevue is beautiful. There’s a lot of food places in the area. The community population is made up of mostly whites, then Asians and Hispanic and then blacks. The community is a well off community because the medium household income is closes to $300,000 per year. Also less than a thousand households receives food stamps. The community however have a high percentage of people that were never married. Even though the population around Bellevue hospital is not widely ethnic, Bellevue caters to a large variety of patients from many different ethnic backgrounds. So of the main health problems of in the community are hypertension, obesity, asthma and tobacco use. Bellevue is also considered a level one trauma center.

Bellevue Hospital Center is located at 462 1st Ave, New York, NY 10016. Bellevue Hospital can be dated back to the year 1736, it is America’s oldest continuously operating hospital. BHC is a member of New York City Health and Hospital Corporation (HHC). Although BHC is located on the east side of Manhattan, it provides services to individuals from 27 different zip codes. The patient population is very diverse, consisting of many different religions, cultures and ethnic background. However, the population of the neighborhood is predominantly Caucasian with the average household income of $106,000. BHC offers a variety of services ranging from medical, surgical, and psychiatric pertaining to both inpatient and outpatient. BHC consist of a 24 story patient care facility housing more then 800 inpatient beds and six intensive care units, along with a world renowned Emergency and Trauma Center. The major health issues that are seen amongst the Bellevue patient population are diabetes, obesity, high cholesterol, hypertension, asthma and tobacco use. On a closing note, BHC is a major health service provider to the large areas of NYC, which has a high demand for access to healthcare.
