My Community project

Objective 1: Demonstrates individual professionalism through personal behaviors and appearances

My approach each day at clinical was as if I was going to work and that is my persona. Also I am aware that I am a role model for my mentee. I am very punctual arriving at school at 8:20 am, ten minutes prior and dressed in a professional attire. Our professor keeps on top of us providing detailed guidelines about our subject on a daily basis. I make it my business to be prepared by reading and doing research on what I need more clarification on. In addition, I will email my professor for further clarification. When I meet with my mentee I am highly confident and competent on my subject matter. I am aware of the standard of practice and clients information confidentiality. In clinical conferences I usually listen well learning from others, and when given the opportunity to speak I share my experience and knowledge with others.

Objective 2: Employ analytical reasoning and critical thinking skills when providing care to individuals and families in the community setting.

In obtaining information from my client I will use interview and the client’s medical records to obtain pertinent information. In practicing holistic care I need to assess the client’s emotional state, cultural, religious, and spiritual impact towards their health. I will continue to gather information if there is a need for assistance in activities of daily living or the client can perform self-care independently. Based on all the data collected I will create a plan of care which prioritize what problem needs to be addressed immediately. My nursing interventions will be put into practice in a safe and timely manner. When administering medication I will follow protocol and use the 5 rights: right patient, right medication, right time, right dosage, and right route. While performing treatment I will also follow specific steps as per protocol and as per physician order. I will evaluate the client’s condition to find out whether the nursing interventions has led to progress or the interventions need modifying. Moreover, I will assess the environment for hazards or potential dangers to myself and the client.  

 

Objective 3: Effectively communicate with diverse groups and disciplines using a variety of strategies regarding the health needs of individuals and families in the community setting.

When interacting with my mentee I have to first assess his communication skills, if he listens, is the message clear, and if I am clear. Does his expression suit his words? Then I decide on the most effective way to speak and reach him. His lack of vocabulary posed a challenge in communicating but we have been working to improve this issue by looking up words together and giving him homework. He has to find the meaning of the words and apply it in 2 sentences. This exercise has benefitted him in expanding his learning and building his vocabulary. I documented what our interaction was, did it meet the specific purpose, and what can be done to improve the flow of effective communication.

 

 

Objective 4: Establish environment conducive to learning and use a plan for learners based on evidence- based practice.

We are based in a government elementary school. When given the opportunity, I will assess the need for learning and development a teaching plan utilizing information from evidence based practice and the use of pictures to stimulate learning. I will encourage my students to study in an environment without distractions, with good lighting, and having the necessary resources to study. I will then evaluate what was our outcome by testing the student.

Objective 5: Utilize informational technology when managing individual and families in the community

I would incorporate nursing information and knowledge with the management of informatics and communication technology for the betterment of the community. I will maintain my responsibility in keeping clients information confidential.

Objective 6: Demonstrate a commitment to professional development.

In planning care for the client I would really emphasize on obtaining my information from the most current information available. I will utilize nursing journals, which in my opinion are most beneficial, and reading textbooks. I am a person who enjoys learning and increasing my knowledge base. Therefore I would implement those attributes to my nursing practice. Today, nursing revolves around technology and more use of evidence based practices. I have taken a personal responsibility to be as current as possible in giving the best care to my client. I am someone who looks introspectively and critically evaluates myself. My biggest challenge is wanting to get tasks done at a certain standard and I find myself having to do it. I am learning how to manage my resource and time and I see the need to delegate some tasks. I have learned to delegate task and hold that person accountable. My strengths are being proactive, flexible, a good team player, and a vigilant leader. The community is always changing and it brings new challenges along with it. I am committed to making the necessary adjustments and being creative to meet the challenges and needs of the client.

Objective 7: Incorporate professional nursing standards and accountability into practice

As a prior member of the military service it is engrained in us to be accountable. Moreover, as a nurse is it is imperative to practice standard of care for the clients. The American nurse’s standard and the agency standard of practice guides a nurse in giving care and treatment to the client. I find it necessary to learn the agency mission statement so my practice are in accordance with their objectives. L

Objective 8: Collaborate with clients, significant support persons and members of the health care team

I will collaborate with other team members of different disciplines, such as physical therapist, doctors, pharmacist, social workers, etc. We will discuss the client’s problem and the necessary interventions to get the client to his equilibrium. We will generate individual care plans and assess the client’s readiness to learn. Once the desire for learning is reached I will teach the client about available health care resources in their community and minor lifestyle changes to achieve their health goal. Furthermore, when the community does not have the necessary resources they will learn to make connection with other communities to meet their needs.

Objective 9:  Recognize the impact of economic, political, social and demographic forces that affect the delivery of health care services

I will make efforts to decrease health care disparities by having health care workers be more culturally sensitive and provide health information that is easy to understand. In addition my colleagues and I will join other community organization and write letters with resident signatures to political leaders. This will address our problem with the gap in health care disparities and the urgency to move swiftly to rectify the problem. Many people who have more resources are provided with better services than those with lesser resources. This is unethical and needs to be addressed by politicians.
