Professional E-Portfolio Reflection for Brandon Archer

 

This E-Portfolio is a reflection of my time at the CNR nursing institution in Brooklyn. It was a very enriching experience as it allowed me to understand the basics of Community Health.

 

Objective 1: Demonstrates individual professionalism through personal behaviors and appearance.

 

EXPECTED BEHAVIORS

 

1.Maintains client confidentiality- During my time at CNR, I did not engage in any behavior that harmed the confidentiality of client(e.g. conveying client information to people who were not members of the healthcare team).

 

2.Assumes responsibility for own learning- I studied my material from my Community Lecture by myself and tried to integrate those theories into practice.

 

3.Prepares for clinical learning- I prepared for the clinical via information from my lecture, observing the clinical environment on a week to week basis, and interacting with my instructor and course-mates.

 

4.Completes assignments within designated time frame- I completed my PowerPoint presentation as well Client Care Plan within their respective timeframes.

 

5.Seeks guidance appropriately- When I was presented with unfamiliar information (e.g. client diagnosis that I did not understand), I asked either instructor Victory-Stewart or one of my course-mates.

 

6. Participates actively in clinical conference- I listened to and engaged in conversation with instructor Victory-Stewart and my fellow course-mates during the semester.

 

7. Attends clinical punctually and in accordance with school policy- For every clinical, I was there before 8:30 a.m.

 

8. Dresses professionally- For each clinical, I was dressed in a shirt and pants. Basically, a business casual attire

 

 

 

Objective 2: Employ analytical reasoning and critical thinking skills when providing care to individuals and families in the community setting.

 

EXPECTED BEHAVIORS

 

1. Uses client interview, nursing and medical records, staff nurses and other health professionals to collect client information- In order for me to do my Client Care-Plan, I collected information via the client(herself) as well as her records.

 

2.Assesses the impact of developmental, emotional, cultural, religious and spiritual influences on the client’s health status- From my interview process, I was able to understand how the client’s demographics influenced her health(e.g. she is a vegetarian)

 

3.Collects significant data relevant to client’s self-care needs- I interviewed my client and reviewed her chart. From those sources, I was able to determine the health needs of the client.

 

4.Completes a physical assessment of selected clients- Due to the nature of the environment, I was not able to perform a complete/thorough physical assessment on the client(e.g. listening to lung & bowel sounds).

 

5.Prioritizes care based on analysis of data- As I completed my Care-Plan for the client, I had to determine which of her health problems were the most urgent(I selected her mobility issues).

 

6. Applies priority-setting in planning nursing interventions- While I did  not perform any hands-on nursing interventions, I did prioritize my planned nursing interventions as I crafted my Client Care-Plan.

 

7.Implements safe, appropriate nursing interventions in a timely manner- Outside of giving verbal advice, I did not perform any actual hands-on nursing intervention at this facility, so this is not applicable to my situation.

 

8. Administers medications and treatments safely- I did not give medications/treatments at this facility, so this is not applicable to my situation.

 

9. Evaluates the outcomes of nursing care- My client verbalized to me that she was following my advice (e.g. following her Physical Therapy regimen).

 

10. Is reflective about practice. Modify client care as indicated by evaluation of client outcomes- From a retrospective view, I believe that my information to the client was relevant and helpful.

 

11.Utilizes principles of personal safety when working in the community setting- I did not engage in any actions that would have produced harm to myself.

 

 

 

Objective 3: Effectively communicate with diverse groups and disciplines using a variety of strategies regarding the health needs of individuals and families in the community setting

 

EXPECTED BEHAVIORS

 

1. Utilizes therapeutic communication skills with individuals and families in the community setting- During my interview process, I engaged in respectful communication with the client and established a trusting relationship with her.

 

2.Utilizes appropriate channels of communication- Since my client is an English-speaking Afro-Jamaican female, I used basic English(verbal skills) to communicate and gain information from her.

 

3. Communicates clearly and effectively with instructor, peers and the health care team- As I presented my Care-Plan, I communicated the health problems about my client to instructor Victory-Stewart and my peers.

 

4.Communicates significant data to instructor and the health care team- As I stated before, any data that was pertinent to my client was presented to the instructor and my course-mates during my Care-Plan presentation.

 

5.Adapts communication skills to the developmental needs of the client- Since my client is English speaking, orientated times 3 and knowledgeable about her health, I did not need any communication modifiers.

 

6. Reports and documents assessments and nursing interventions accurately- I provided accurate information about my assessments and nursing interventions(even though they were not performed in actuality) via my Client Care-Plan.

 

 

 

Objective 4: Establish environment conducive to learning and use a plan for learners based on evidence-based practice.

 

EXPECTED BEHAVIORS

 

1.Develops and implements a teaching plan for an adult and/or family in the community setting- The purpose of my Client Care Plan was to identify and respond to the health needs of my client.

 

2.Establish environment conducive to learning- I established a trusting and respectful relationship with my client.

 

3.Evaluates client/family learning outcomes- From receiving feedback, it seems like she learned from the health information that I provided to her.

 

 

 

Objective 5: Utilize informational technology when managing individual and families in the community.

 

EXPECTED BEHAVIORS

 

1.Utilize principles of nursing informatics in the clinical area- At CNR, we did not utilize any nursing informatics or computer-related skills, so this is not applicable.

 

2.Maintain strict confidentiality with client records- In respect to the abovementioned question(#1), this is also not applicable.

 

 

 

Objective 6. Demonstrate a commitment to professional development

 

EXPECTED BEHAVIORS

 

1.Uses appropriate current literature in planning care for clients in the community setting- As I crafted my Care-Plan, I utilized current information from my Community lecture as well as relevant non-course related materials.

 

2. Assumes responsibility for lifelong learning- As a registered nurse, I understand that I would be continually learning new skills and theories for the duration of my nursing career.

 

3. Engages in self-evaluation- After every clinical, I retrospectively looked over items that I accomplished with my client and entities that I did not meet or that needed improvement.

 

4. Is committed to adjusting to the challenges of independent practice in community health nursing- From this experience, I realize and respect the work that community health nurses do in regards to helping communities that might not have the best resources available to give to their occupants.

 

 

 

Objective 7. Incorporate professional nursing standards and accountability into practice

 

EXPECTED BEHAVIORS

 

1. Utilizes American Nurses Association Standards in clinical practice- During my tenure in this clinical, I did everything in my power to uphold the standards of the nursing profession as proclaimed in the American Nurses Association Standards of practice.

 

2. Complies with agency standards of practice- I followed the standards of practice that this facility had in place.

 

3. Is accountable for actions in the clinical area- As a registered nurse, I know that I am accountable for my actions in any clinical area

 

4. Is aware of the assigned agency’s mission- I was able to view the mission statement of the organization and follow it throughout my tenure at the facility.

 

 

 

Objective 8. Collaborate with clients, significant support persons and members of the health care team

 

EXPECTED BEHAVIORS

 

1. Collaborates effectively with health care team to address client problems- As I stated earlier, I mentioned the needs of my client to the health team (instructor, etc.)

 

2. Coordinates client-care based on client needs and therapeutic interventions- In terms of coordinating any sort of institutional and/or discharge care, I did not do this procedure.

 

3. Identifies health care resources for client/families- Similar to the question above, I did not engage in any sort of case management procedures.

 

4. Guides clients/families to make appropriate lifestyle and treatment choices- I told the client the importance of maintaining her physical therapy schedule in order to improve her mobility as well as educating her about adequate bowel habits(high fiber diet and so forth).

 

5. Assist clients to make connections to other community agencies- I did not make any actual connections to any outside community agency during my time with my client, so this is not applicable to my situation.

 

 

 

Objective 9. Recognize the impact of economic, political, social and demographic forces that affect the delivery of health care services

 

EXPECTED BEHAVIORS

 

1. Recognize gaps in care system- Honestly, even though this facility serves an older adult population composed mostly of racial minorities in an inner-city district, their service from the naked eye seems sufficient to the needs of the clients.

 

2. Begin to identify solutions to complex problems in the clinical area- I honestly did not witness and/or experience any complex problems at this clinical site.

 

3. Acts as change agent in advocating to appropriate health care resources for client/families-In my tenure at CNR, I honestly tried to give my client valuable tips on how to improve her health.

 

 

 

Summary

 

I enjoyed my tenure at the CNR facility. I was able to collaborate with my fellow course-mates and gain valuable information from instructor Victory-Stewart. At CNR, I saw the importance of having a team approach (nurses, social workers, CNAs, etc.) and its positive influence on client care. Finally, I realize the importance of community based health care as more healthcare programs are moving away from the hospital(acute-care) setting to the home/community based setting.
