Congratulation to the acceptance to Cornell and Hospital of Special Surgery. In the meantime please see attached for Private Duty requirements, physical form, and 2 reference forms (to be filled out by supervisor or co-worker) to the email. Make sure your doctor attached lab reports to the fields that required the report (Rubella, Rubeola, varicella, hepatitis B and C, chest x-ray if PPD was positive and drug screen reports). After ALL the documents have been obtained, please email me and schedule an appointment to meet no later than October 17th to be guarantee a spot in the orientation . The next orientation date will be October 21st . Please see attachments for requirement information and get ALL the paperwork in compliant before the orientation date. You will have to come in to the office with all paperwork for an interview before being sign-up for orientation by the coordinator. Thank you.
