Patient: S.G.   34yo female BP 107/78 PR 71.

Medical history WNL. Smokes cigarettes about a pack a day ASAII.

EO/IO- NSF

Dental- few restorations, caries on #3, 5, 9, 18, 29, and 31. Retained root tips of #2 and #30. #17 partially erupted. Patient presents with generalized erosion and cervical 3rd caries. Recession due to  labial frenum pull on #24.

Perio- Type II  with localized type IV, #24 requires graft. Gingiva is red, moderate marginal inflammation, bulbous and shiny.

OHI- patient taught how to use end tuft brush on mandibular anteriors, recommended colgate sensitive tooth paste for recession.

Referral given for caries evaluation and oral surgeon.

 

 

Patient K.M.- Male 36 yo BP 156/97 PR71, ASAII uncontrolled hypertensive.

EO/IO- NSF

Dental- multiple restorations, localized abrasion,  recurrent decay on #30 and #31. #16 is impacted with possible odontoma or supernumerary tooth.

Perio- Type II 3-6mm BOP. Gingiva is pigmented with slight marginal inflammation, localized flaccid tissue on mandibular anteriors, and shiny.

Referral given for physician patient needs blood pressure evaluation, caries evaluation and oral surgeon.
