Case Study #1

Initial visit: 40 years old male, non smoker, BP 140/91

This patient last physical examination was around 6 years ago. He was not taking any medications and he stated he had none systemic conditions. Intra/Extra oral examination was within normal limit. This patient was classified as type II periodontitis because he presented with generalized recession, CAL ranging from 3-5mm, mobility grade 2 (1-2mm) on teeth #24 and #25 and moderate bleeding upon probing. Patient also showed generalized attrition mostly on anterior teeth. Patient had heavy sub gingival deposits with medium stain on occlusal and lingual surfaces.

Description on his gingiva was as followed: generalized, red, inflamed and soft gingiva. A referral was given for periodontal evaluation and high blood pressure. Treatment plan at this visit was to take a full mouth series, use a power tooth brush, daily use a moderate interdental brush because he had embrasure type II (about 75% of embrasure is occupied by the gingiva) and rinse 2x day with antiseptic rinse. Recall visit in 3mths due to periodontitis and heavy case.

Case Study #2

61 years old. BP 126/85 Pulse 60. No systemic conditions. ASA 1.

Patient’s last dental cleaning was 15 years ago.

EO/IO no significant findings.

Patient is missing teeth #3,5,12,18,19 and 31. Generalized attrition and erosion. Cavities on teeth #1,9,11 and 16. Perio type II 4-5mm pocket depths and localized 7mm on lower anteriors from #22-27. Generalized deposits mostly found on interproximals and supra-gingival calculus on anterior linguals. Generalized red, inflamed, soft gingiva with severe BUP. 

Patient calculus was tenacious with minimal staining.

Recommended FMS and four quadrant scaling.

OHI interdental moderate brush due to size of embrassure or woven floss.

A referral form was given to the patient for periodontal evaluation.

Full Mouth Series were taken and all quadrants were scaled using ultrasonic and hand instruments. Recall in 3 months.
