DEMOGRAPHICS
Patient is L.P. Age 34, Heavy / Type III.
ASSESSMENT
Patient has no significant medical history, allergies, surgeries or hospital visits.
Vital signs within normal limits.
Patient is a non-smoker and a social drinker.
Patient not currently taking any prescription medications or vitamins.
PERIODONTAL
Patient had generalized moderate gingival inflammation. Patient had probing depths ranging from 1mm-4mm with localized probing depths of 5mm and 6mm. Bleeding upon probing.
RADIOGRAPHS
Panoramic radiograph exposed 10/2017 and utilized.
Bitewing radiographs exposed and utilized 4/2018.

EVALUATION
Upon return, all Arestin site displayed an improvement in probing depths.
